Key Responsibilities

• Provide skilled physical therapy services to adult patients in a home health setting across the Montgomery and upper Prince George's County areas
• Conduct evaluations, develop individualized treatment plans, and deliver therapeutic interventions during home visits
• Manage a caseload of approximately 6 patient visits per day
• Maintain accurate and timely clinical documentation in accordance with home health standards
• Collaborate with interdisciplinary care teams to support patient goals and continuity of care

Required Qualifications

• Valid Maryland Physical Therapist state license required at time of submission
• Current BLS certification required
• Minimum of 1 year of physical therapy experience
• Home health experience required
• Two supervisory references required

Preferred Experience / Skills

• Experience with HCHB (Homecare Homebase) documentation platform preferred
• Familiarity with adult home health case management and care coordination
