Discharge & Care Coordination Case Manager

Memorial Hermann Health System is seeking a dedicated Case Manager to coordinate patient care progression and manage all aspects of discharge planning. The ideal candidate will be a graduate of an accredited nursing program or hold a Master of Social Work, with experience in utilization management and strong interpersonal skills.
