Speech-Language Pathologist (SLP) Citrus Heights Post Acute – Citrus Heights, CA. $60.00 – $75.00 per hour (DOE). $10,000 Sign-On Bonus (Full-Time). Make a Meaningful Impact in Post-Acute Care. Citrus Heights Post Acute is seeking a compassionate and driven Speech-Language Pathologist (SLP) to join our skilled nursing team. In this role, you’ll help patients improve communication, swallowing, and cognitive function while working alongside a collaborative interdisciplinary team. Key Responsibilities Evaluate patients within 24 hours (or 1 business day) of physician referral Develop and implement individualized treatment plans Provide speech therapy services in accordance with physician orders Monitor patient progress and adjust treatment plans as needed Maintain accurate, timely documentation in EMR systems Collaborate with physicians, nursing staff, and rehab team members Participate in care conferences, rehab meetings, and discharge planning Educate patients, families, and caregivers on home programs and ongoing care Provide staff training and in-services as needed Qualifications Master’s or Doctorate degree in Speech-Language Pathology Active California SLP license (required) Strong clinical, communication, and documentation skills Skilled nursing or post-acute experience preferred (not required) Ability to thrive in a fast-paced, team-oriented environment Certifications & Requirements Current SLP license in good standing Ongoing continuing education per state requirements Experience with or willingness to learn EMR systems Why Join Citrus Heights Post Acute?
