Geriatrics Nurse Practitioner

A well-established senior care community in Freehold, NJ is seeking a compassionate and experienced Nurse Practitioner to provide high-quality, patient-centered care to a geriatric population. This role spans primary care, subacute, long-term care, and assisted living settings , offering a dynamic and meaningful clinical environment.

This is an excellent opportunity for an NP who thrives in a collaborative yet autonomous role , as you will serve as the sole provider onsite during scheduled days while working closely with an interdisciplinary care team.

• Monday, Wednesday, Friday 8:30 AM 5:00 PM
• Option to add Tuesday or Thursday at an additional nearby location
• Flexibility for part-time or expanded full-time schedule

• Conduct comprehensive assessments and physical exams for senior residents
• Diagnose and manage acute and chronic conditions
• Develop and oversee individualized care plans
• Prescribe medications and treatments as appropriate
• Provide education and counseling to patients and families
• Collaborate with physicians, nurses, and interdisciplinary teams
• Maintain accurate and timely documentation in EMR
• Participate in care team meetings and quality initiatives
• Support efforts to reduce hospitalizations and readmissions
• Perform IV insertion as needed (infrequent)

• Geriatric patients with complex, multi-morbidity conditions
• Mix of subacute, long-term care, assisted living, and primary care visits
• Average patients per day ( 2 3 per hour)
• Smaller, close-knit setting with 200 residents
• Strong emphasis on patient experience, bedside manner, and family communication

• Master s Degree in Nursing (MSN) from an accredited program
• Active New Jersey Nurse Practitioner license
• 5+ years of experience in primary care or geriatrics
• Recent (within 3 years) subacute or acute care experience
• Strong clinical judgment and knowledge of geriatric care
• Ability to work independently and collaboratively
• Excellent communication and interpersonal skills

• FNP or AGPCNP certification
• Experience in long-term care or medical group settings
• Willingness to travel between nearby locations (mileage reimbursed)
