Experience: Minimum 2 years

License: Bachelor's degree in social work or related field required

Certifications: GRH CPI/MOAB

Description: Provide psychosocial support to patients and their families. Assist vulnerable populations coping with chronic, acute, or terminal illnesses. Address social, emotional, and financial needs of patients. Conduct clinical assessments and crisis interventions. Provide brief therapy and case consultation with the healthcare team. Collaborate with other professionals in the community. Advise family caregivers regarding patient care. Offer patient education and counseling. Make necessary referrals to community resources for continuity of care. Assist family members in understanding and supporting the client or patient. Collaborate with the discharge planning team to facilitate appropriate discharge needs. Meet standards set by regulatory agencies. Average case load per social worker is 15. Weekend rotation every 6th weekend required. Holiday rotation required.
