Position Overview Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a home health assignment in Valley City, ND . This role involves providing therapy services to patients in their homes across a 60-mile radius , treating a mix of post-surgical, fall recovery, and general home health cases. Key Responsibilities Conduct in-home patient evaluations and develop care plans Provide skilled PT treatment to home health and hospice patients Manage a caseload of 5–7 patients per day Educate patients and families on mobility, safety, and recovery Coordinate care with interdisciplinary teams Requirements Active North Dakota PT License Minimum 1 year of Home Health experience REQUIRED BLS Certification (AHA) Valid Driver's License & Auto Insurance REQUIRED Strong time management and independent work skills Schedule: Monday–Friday | 8:00 AM – 5:00 PM 40 hours guaranteed Coverage area: ~60-mile radius No interview required (fast submission process) Must live 50+ miles from facility Cannot have worked with CommonSpirit/CHI/Dignity within past 12 months Facility may cancel up to 4 shifts per contract #J-18808-Ljbffr Trinity Healthcare Talent Services, LLC
