Contract | 13-Week Assignment | Skilled Nursing Facility (SNF) | 35-Hour Weekly Guarantee
Key Responsibilities
- Evaluate and treat patients in a skilled nursing facility setting to restore and improve physical function
- Develop and implement individualized physical therapy treatment plans based on patient needs and goals
- Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
- Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
- Educate patients and families on therapeutic exercises and home care strategies
Required Qualifications
- Active Physical Therapist (PT) license in the state of Virginia or ability to obtain prior to start
- Graduate of an accredited Physical Therapy program
- Current CPR/BLS certification
- Experience working in a skilled nursing or long-term care setting
Preferred Experience / Skills
- Previous travel or contract therapy experience
- Familiarity with MDS documentation and Medicare guidelines in a SNF environment
