This is a remote role with up to 75% field travel, MondayFriday from 8:00 AM5:00 PM EST./ppIn this position, you will play a key role in delivering high-quality, cost?effective care. Youll leverage your clinical expertise to develop individualized care plans, coordinate physical, behavioral, and psychosocial services, and ensure members receive the support they need to thrive./ph2Key Responsibilities/h2ulliConduct in-home, face-to-face assessments for members enrolled in:/liulliManaged Long-Term Services and Supports (MLTSS)/liliDual Special Needs Programs (D-SNP/FIDE)/li/ulliEvaluate member needs and coordinate services such as:/liulliAdult/pediatric medical day care/liliPersonal care assistant services/liliNursing facility placement/liliMLTSS program enrollment/li/ulliPartner with members, caregivers, PCPs, and interdisciplinary teams to develop and implement individualized care plans./liliAuthorize services in alignment with program guidelines and support safe transitions of care./liliDocument all case activity accurately and promptly in the electronic health record./liliApply strong clinical judgment and critical thinking to manage complex care needs./liliMentor new team members once proficiency is achieved./li/ulh2Required Qualifications/h2ullipActive, unrestricted RN license in New Jersey/p/lilipComfort conducting field?based home visits across all socioeconomic levels and community settings, including any neighborhood or environment, and with members in a wide range of living conditions or circumstances./p/lilip3+ years of clinical practice experience/p/lilip3+ years of experience in home health, hospice, or a similar setting/p/lilipAbility to manage pediatric and medically complex cases/p/lilipResidency in Essex County NJ/p/lilipSuccessful completion of the NJ Choice Certification, including:/p ullipo Minimum exam score of 80%/p/lilipo Completion of state training modules/p/lilipo Field mentoring component/p/li/ul/lilipMust possess reliable transportation and be willing and able to travel up to 75% of the time. Mileage is reimbursed per our company expense reimbursement policy/p/li/ulh2Preferred Qualifications/h2ulliExperience with LTSS/liliCase management and/or discharge planning background/liliManaged care experience/liliFamiliarity with Medicaid and Medicare/liliProficiency using multiple systems and MS Office Suite (Outlook, Word, Excel, SharePoint, Teams)/li/ulh2Education/h2ulliMinimum of an Associate Degree in nursing/li/ulpAnticipated Weekly Hours/pp40/ppTime Type/ppFull time/ppPay Range/ppThe typical pay range for this role is:/pp$66,575.00 - $142,576.00/ppThis pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
