WCS Healthcare is partnering with a large healthcare organization to hire experienced Registered Nurses (RNs) for a Hybrid Clinical Assessment Coordinator opportunity throughout the Westchester County Region. This role combines field-based patient assessments with remote documentation and care coordination responsibilities.

Schedule & Compensation
- Full-Time
- Hybrid Schedule: 3 Days Field-Based / 2 Days Remote
- Monday-Friday

Responsibilities
- Conduct initial in-home assessments for patients and members.
- Review medical records, physician referrals, and supporting clinical documentation.
- Complete and accurately enter assessments into the UAS-NY system.
- Ensure assessments comply with New York State guidelines and documentation standards.
- Identify clinical concerns, discrepancies, and care needs during evaluations.
- Educate patients and families on available healthcare services, community resources, and managed care options.
- Support enrollment into appropriate managed care programs.
- Collaborate with nurses, social workers, providers, and interdisciplinary team members to improve care coordination.
- Maintain detailed case notes and assessment documentation.
- Participate in quality assurance reviews and comply with regulatory requirements.
- Promote health maintenance, disease prevention, and continuity of care initiatives.
- Stay current with changes in assessment regulations and healthcare policies.
Qualifications
- Active New York State Registered Nurse (RN) License
- Minimum 2 years of Home Care experience
- Experience completing clinical assessments in the community preferred
- Knowledge of UAS-NY strongly preferred
- Excellent assessment, documentation, and communication skills
- Reliable transportation and ability to travel throughout the Bronx service area
