Job Summary This position works with limited oversight to efficiently provide nursing triage of all patient messages received via telephone or MyChart. The triage nurse will demonstrate sound clinical judgment that utilizes careful listening, critical thinking and assessment skills. During downtime, the position works with limited oversight to efficiently process patients to appropriate units based on physicians' orders whether coming via ED, DA, Transfer, Clinic, Surgery or Procedural Area. Required Qualifications Registered Nurse licensure by the Texas Board of Nurse Examiners Preferred Qualifications 3–5 years of ambulatory clinical nursing experience or emergency room experience Telephone triage experience (strongly preferred) Job Duties Utilize strong listening skill to interpret subtle indications of the patient's complaint and form a thorough assessment without visual observation. Follow clinic‑specific protocols and/or professional judgment to identify the nature and urgency of patient needs and determine the appropriate disposition. Provide patient education and advice to improve continuity of care, avoid unnecessary visits to the emergency room, and reduce risks from medical complications. Collect sufficient data related to the presenting problem and medical history, recognizing symptom patterns to provide safe, timely disposition. Refill medications per clinic‑specific protocols. Maintain ongoing communication with internal stakeholders and external customers while securing the patient's admission or internal transfer. Apply clinical expertise and the nursing process to triage and prioritize admission acuity from multiple locations (direct admits, external transfers, ED, onsite clinics, private physician offices). Ensure smooth functioning of the patient placement process to reduce wait times and avoid unnecessary movement of patients, serving as an expert clinical resource. Facilitate the direct admit process by interacting with physicians to ensure telephone triage results in appropriate patient placement. Notify the Administrative Coordinator or Manager concerning admissions that do not meet the hospital's criteria. Continuously seek new ways to improve processes and increase efficiencies, and communicate recommendations to the Administrative Coordinator or Manager. Perform other duties as assigned.
