Overview Physical Therapist, General, Concord, New Hampshire. Start date 09/21/2026. Travel - temporary assignment for 15 weeks with guaranteed 40 hours per week. Shift: 8-hour daytime days, five days per week. This outpatient rehabilitation services role supports a satellite outpatient site, working with post-operative and other outpatient patients as part of an integrated care team. EMR: Connect. Key Highlights Weekly pay: $2,192 to $2,199 per week Guaranteed hours: 40 per week Contract duration: 15 weeks Shift: 8-hour daytime shifts, five days per week Start date: 09/21/2026 Setting: outpatient rehabilitation services at a satellite outpatient site Responsibilities Provide evaluation, treatment planning, and ongoing physical therapy care for outpatient clients Implement and monitor individualized treatment plans and adjust as needed Manage care for post-operative patients requiring rehabilitation and functional restoration Perform routine assessments using standard tests and scales; document progress in the EMR Verify and apply relevant orders and clinical data (vital signs, labs, etc.) as part of patient care Educate patients and families on exercises, safety, and home programs; ensure patient and caregiver understanding Requirements Active New Hampshire or Compact PT license or willingness to obtain a temporary license Outpatient therapy experience; comfortable seeing post-operative patients Ability to work independently with strong clinical and learning agility Proficiency with EMR systems or ability to learn Connect quickly Excellent communication skills and a collaborative approach to care Benefits and Support Dedicated support from Healthforce Healthcare LLC throughout the assignment 24/7 recruiter and travel support Onboarding and credentialing assistance EMR orientation and training to support rapid integration
