Full-Time Opportunity for a Post-Acute Care Physician

Our organization is in search of a full-time Geriatrician to become a vital member of our collaborative academic geriatrics program. This role focuses on delivering exceptional post-acute and long-term care at community-based locations throughout the Atlanta metropolitan area.

This innovative, team-oriented approach integrates clinical practice, education, and a commitment to enhancing outcomes for patients with complex medical needs, ensuring a unique clinical service that fosters continuity of care and positively influences patient recovery and overall quality of life.

The preferred candidate will hold Board Certification or be Board Eligible in Geriatrics, Family Medicine, or Internal Medicine, with a background in managing complex care and working within multidisciplinary teams.

Practice Overview

• The service line encompasses post-acute rehabilitation, sub-acute care, and long-term care across three community sites affiliated with our organization, totaling approximately 500 beds.
• The patient demographic primarily consists of older adults, with around 60% of the population being post-acute, and an average length of stay of about three weeks.
• A growing multidisciplinary team includes over five physicians and highly skilled Advanced Practice Providers (APPs) to address acute issues, manage medically complex patients, and provide ongoing care, alongside nurses, physical therapists, nutritionists, and more.

Position Highlights

• Engage in a collegial atmosphere that promotes a strong team culture and interdisciplinary collaboration.
• Deliver full-time clinical services at one or two community sites, managing an average patient load of 11-13 per day.
• Oversee a combination of post-acute care, including admissions, transitions, and discharge planning, as well as long-term primary care involving consultations, referrals, and medication management.
• Work Monday through Friday, with potential for flexible scheduling, and participate in a low-volume telephone call rotation shared among more than ten physicians in the geriatrics group.
