Outpatient Physical Therapist

The Outpatient Physical Therapist will evaluate assigned patients and develop, implement, and modify individualized treatment plans in accordance with patients' needs, goals, and physician direction. The therapist will be responsible for the delegation and direction of discipline-specific therapy services for patients, ensuring the design of functional treatment programs that promote a successful return to community activities.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop treatment plans and goals focused on progressing patients towards discharge.
• Document medical records accurately and timely, including goal writing and functional quality indicator assessments.
• Provide therapy services under valid physician orders and within insurance approval.
• Participate in student programs, facilitating learning through supervision and evaluation.
• Engage in professional development through continuing education and self-study.
• Adhere to the State Practice Act and maintain professional licensure in good standing.
• Contribute to organizational performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• Minimum of 1 year of outpatient clinical experience.
• Current CPR certification.

Schedule

This position operates on a 5x8.0 schedule.
