Job Description Green Meadows Home Health is seeking a Licensed Vocational Nurse (LVN) to join their Home Health Case Manager team. The position is full‑time, Monday through Friday, 9:00 am to 5:30 pm. Full‑time employees receive PTO, sick time, holidays, health insurance, and 401(k). Responsibilities Intake and referral of new home‑health cases, verifying eligibility and uploading required documentation. Enter and complete all required fields on patient profiles, flagging disciplines when necessary. Initial staffing of cases and coordination with care coordinators/managers regarding SOC staff and dates. Respond to intake/referral emails, validate patient profile completion, and ensure all fields are addressed. Coordinate discipline tasks for the SOC, validating that the SOC discipline is scheduled for the correct date and that supplies are available in the patient’s home prior to arrival. Notify patients and families of home‑health services, collect medication information, and provide ID and insurance card details to the discipline. Email a short report to the discipline summarizing the patient’s findings, visit frequency, and entered medications. Respond to all emails addressed to the case manager. Ensure authorizations are obtained and staffing is completed throughout the episode of care. Monitor MVR compliance and ensure no issues arise throughout the episode. Check supplies and medications weekly, delivering replenishments by Friday whenever possible. Manage lab and DME orders, ensuring all results and orders are documented in patient charts and communicated to staff. Qualifications Licensed Vocational Nurse (LVN) license required.
