Certified Occupational Therapist Assistant ~ COTA

Mantey Heights Rehabilitation & Care Center, part of the Stellar Living family in Grand Junction, CO, is seeking a compassionate and dedicated Certified Occupational Therapist Assistant (COTA) to join our Healthcare & Medical Services team. In this role, you will work under the direction of an Occupational Therapist to provide high‑quality rehabilitative care to residents in our assisted living, memory care, and independent living community. You will carry out individualized treatment plans designed to improve residents’ functional abilities, safety, and independence in daily activities, always aligning your care with our culture of family, wellness, and engagement.

As a COTA at Mantey Heights, you will conduct therapeutic activities and exercises, support residents with self‑care and mobility training, and adapt activities and environments to help them maximize independence and quality of life. You will carefully observe and document resident responses to treatment, report progress and concerns to the Occupational Therapist, and collaborate closely with nursing, caregivers, and families to ensure coordinated, resident‑centered care. Your day may include working with residents recovering from illness or injury, those managing chronic conditions, and individuals living with cognitive or memory challenges.

You will be responsible for following all established treatment protocols, infection control and safety guidelines, and regulatory standards while maintaining accurate and timely documentation within the medical record and therapy systems. You will assist with screenings, contribute observations to care planning and interdisciplinary meetings, and provide education and encouragement to residents and their families regarding home exercises, adaptive strategies, and safe participation in daily tasks. Consistent, clear communication, professional behavior, and respect for residents’ dignity and preferences are essential.
