Home Care RN Case Manager

Community Health Network has been placing experienced nurses and physical therapists in home care, hospice, and case management for over 30 years. We offer a stable, supportive, and growing organization that focuses on your needs and interests. Currently we are assisting one of our top home care agency partners with a need for an experienced Home Care RN Case Manager to see patients in homes in the Hopkinton/Southboro/Framingham/Ashland MA area. Full-time, 40 hours/week, Monday-Friday Occasional weekend rotation may be required No on call Annual Salary Range $125,000 - $145,000 (based on experience) Mileage reimbursement Requires minimum 1 year of recent Home Care RN Case Management experience doing OASIS visits with a Medicare certified agency. The Home Care Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge.
