PT - Physical Therapist

The Outpatient Physical Therapist evaluates patients, develops, implements, and modifies treatment plans according to patient needs, goals, and physician direction. This role includes the delegation and direction of all therapy services for assigned patients, ensuring the design of functional treatment programs for effective community reintegration. Responsibilities Conduct comprehensive evaluations of assigned patients. Utilize clinical reasoning to develop treatment plans and goals focused on patient discharge within an established length of stay. Document medical records clearly and concisely, including goal writing and functional quality assessments. Provide care under signed, valid physician orders and within insurance approval as necessary. Participate in student programs, offering a positive learning environment with appropriate supervision. Create and implement a professional development plan, pursuing continuing education and self-study. Adhere to discipline-specific State Practice Acts and maintain professional licensure in good standing. Demonstrate awareness of organizational performance improvement initiatives and contribute feedback for enhancement outcomes. Required Experience / Certifications / Licensure Degree in Physical Therapy from an Accredited School. 6 months to 1+ year of outpatient clinical experience. Current CPR certification. Active and applicable state license. Previous assignments with Select Medical are highly desired. Schedule This position follows a 5x8 schedule, with six-month contract duration desired.
