Case Worker

The Caseworker is responsible for assessing client needs and connecting adults, children, and families with appropriate community, healthcare, and government resources. This role involves building relationships with clients, coordinating services, providing guidance on available assistance programs, and helping individuals access the support needed to achieve positive outcomes. The ideal candidate is compassionate, resourceful, and committed to improving the lives of others through advocacy and support.

Minimum Qualifications

• 1+ year of professional experience in case management, social services, healthcare, or a related field preferred

• Bachelor's degree in Health Sciences or a related field preferred

• Familiarity with Medicaid programs, guidelines, and regulations preferred

• Current CPR certification (if applicable)

• TB questionnaire, PPD, or chest X-ray completion required (if applicable)

• Must meet all federal, state, and local employment requirements

• Must be at least 18 years of age
