Patient Population: Geriatric (55+ Population)
Setting: PACE / Outpatient Community-Based Care
Responsibilities: Medical direction and supervision, participant care, interdisciplinary collaboration, quality oversight
Required Procedures: Physical exams, reassessments, chronic disease management, diagnostic evaluations
EMR System: Not Specified
Support Staff: PCPs, APPs, Nursing Staff, Interdisciplinary Team
Days Per Month: Full-Time
On-Call/Weekends: On-call rotation as required

Submission Requirements:

Active North Carolina Physician License Required
Board Certified in Internal Medicine or Family Practice Required
CDS/DEA Required
Minimum 1 year geriatric/frail elderly experience Required
