Qualifications Licensure: Valid LGSW or LISW license in Minnesota (Master’s‑Level Social Worker). Experience: At least one year of clinical Social Work experience; proficiency in Electronic Medical Records (EMR) preferred. Responsibilities Conduct home visits to assess patients’ psychosocial status and needs. Provide a range of social work services in the home, including short‑term individual counseling, community resource planning, and crisis intervention.
