About the Opportunity Ahwatukee Post Acute is growing — and we are seeking a dedicated PhysicalTherapist to join our in-house therapy team! Our 192 bed facility is currently undergoing a major renovation, including construction of a state - of-the-art 44bed behavioral and dementiacare unit, enhancing our ability to provide specialized services to the Phoenix and Ahwatukee communities. We proudly maintain a CMS 5-Star Quality Measure rating and foster a supportive, collaborative clinical environment focused on patient-centered success. What You'll Do Perform skilled PT evaluations and provide evidence-based treatment Develop individualized care plans to increase function, safety, and mobility Document patient progress using NetHealth Collaborate closely with nursing and IDT using PointClickCare Educate patients, families, and caregivers on therapy strategies You'll Work With: 1 other Full-Time PT Multiple Full-Time PTAs 2 Full-Time OTs & multiple Full-Time COTAs Full-Time onsite SLP A strong and fully staffed in-house therapy department — no contracting or travel required Qualifications Arizona PT license (or eligible prior to start date) Strong communication and teamwork skills A passion for working with the geriatric and memory care population New grads welcome and supported!
