Community Resource Advocate & Care Navigator
BIGHORN VALLEY HEALTH CENTER
One Health is seeking a Community Resource Specialist to help patients access essential services, benefits, and support. You will connect clients with providers, coordinate care, and advocate for needed resources in a fast-paced, compassionate team. The role includes cross-training as an Intake Coordinator to cover for short staffing, with a 40-hour work week, typically Monday–Friday, 8am–5pm.
