As supervised and delegated by a registered nurse, provides direct patient care to patients in accordance with applicable scope and standards of practice.

How you'll contribute

Under the direction of an RN, contributes to the assessment of patients by collecting data for analysis by an RN Essential Functions

Coordinates PCMH training activities. Completes data analysis to drive efforts to meet quality benchmarks. Serves as a role model in PCMH by developing, educating, and training clinic staff/providers on quality measures, PCMH concepts and reporting. Performs activities related to the collection, processing and analysis of quality data to assure accuracy and reliability. Identifies and prioritizes quality measure performance needing improvement. Nonessential Functions Assist with orientation and training of personnel and students. Participates and attends meetings as required and/or assigned. Prepares monthly reports and as requested.

What we're looking for

Minimum Education High school diploma or equivalent required

Required Skills Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.

Certifications Basic Life Support (BLS) Licenses: Current LPN license in state of NC

Minimum Work Experience 1 year Electronic Medical Records experience 1 year Patient Centered Medical Home (PCMH) experience 1 year quality review/ quality management experience 3 years ambulatory care experience (Preferred)

Active North Carolina LPN license (single-state) or compact required. If relocating, licensure must be updated to the new Primary State of Residence (PSOR) before employment begins
