Job Description

We are seeking a skilled home care professional with a minimum of one year of experience in home health care. This role involves providing medical care in patients' homes, focusing on a variety of diagnoses and requiring specific medical skills.

Required Skills

• Home health experience with OASIS training
• IV skills and wound care experience
• Familiarity with general medical diagnoses
• Experience with high-risk diagnoses such as heart failure, COPD, myocardial infarction, pneumonia, stroke, and post-operative care

Certifications Needed

• BCLS Certification

Job Responsibilities

• Conduct cardiopulmonary assessments
• Provide basic wound care, including ABI compression dressing and wound VAC management
• Perform Pleurx/Asept drainage care
• Handle foley catheter changes and care
• Remove staples and sutures as required
• Utilize glucometers and pro-time machines
• Perform supervision and administration of SQ/IM injections
• Carry out lab draws and venipunctures
• Manage IV skills and PICC line or midline care and dressing changes
• Perform SQ port access and deaccess
• Complete home skilled visits, typically ranging from 6 to 8 per day

Additional Requirements

• Valid driver's license and personal vehicle for home visits
• Compliance with COVID-19 vaccination or valid exemption form on file
• Flu vaccination or declination required, along with mandatory mask wearing while at work
• Flexibility with scheduling, as visit times may vary based on office needs
• Must have a minimum of one year of relevant experience

Work Environment

This position involves performing skilled home care visits in various patient homes, primarily catering to adults and geriatric populations. The workload includes collaboration with an interdisciplinary team, including physical therapy, social services, and pharmacy.

Patient Care Details

Daily patient census can range between 400 and 1000 at multiple locations. Expected patient care includes handling a variety of diagnoses, including general medical and high-risk cases.
