Pharmacist in Van Nuys, CA

A dynamic opportunity is available for a pharmacist specializing in Transitions of Care/Ambulatory Care within a large, 400-bed medical center located in the San Fernando Valley region. This role is ideal for those passionate about improving patient outcomes through direct involvement in medication management at a critical point in patient care-the transition from hospital to home.


Responsibilities include working closely with patients prior to discharge to provide thorough medication education and counseling. This pharmacist will assist with medication refills before patients leave the hospital, supporting adherence to prescribed regimens and contributing to healthier post-discharge outcomes. This role is integral to the 'meds to beds' program, ensuring patients have the medications they need in hand, reducing potential complications.


Preferred candidates are recent PGY1 pharmacy residency graduates, though applications from experienced pharmacists without residency training will also be considered.

• Within a LA County Department of Health hospital system

This position is offered as a long-term contract through a staffing agency but functions within the infrastructure of a large public health system. Pharmacists have the opportunity to later apply for permanent roles within the County health system.
