Job Title: RN Case Management Profession: RN Specialty: Case Management Duration: 13 Weeks Shift: Day Shift Hours per Shift: 8:00 AM - 4:30 PM Experience: Minimum 2 years in a hospital setting License: Active RN License in New Hampshire Certifications: BLS Required Must-Have: - 2 years experience in a hospital setting required - Active compact license at submission - NH RN license at time of submission Description: Seeking an RN for a case management role. Candidates must have acute hospital RN case management experience. Experience in critical care is beneficial. Responsibilities include utilization management, care coordination, and discharge planning. Candidates should have leadership qualities. Dress code is business casual. Patient ratios on the unit are 18 to 1. There are 36 beds in the unit with a team consisting of 2 staff members. Weekend rotation is required for day shift.
