Care Coordinator

Role Care Coordinator RN (CC RN) responsible for assessing transitional care needs, coordinating care across the continuum, and engaging with patients and families to ensure care goals are met. Responsibilities Conduct psychosocial and functional assessments, develop individualized discharge plans, facilitate interdisciplinary communication and care progression, and address barriers to timely discharge. Collaborate with care team members, physicians, and patients to support care transitions, manage documentation accurately, and participate in rounds and care conferences. Ensure resource availability, monitor care delays, and facilitate post-acute referrals. Qualifications & Skills Minimum of 1 year nursing experience in acute care, RN licensure (single or multi-state), BLS certification, excellent communication, organizational, and problem-solving skills. Ability to work collaboratively, handle stress, and adapt in a fast-paced environment. Knowledge of regulations and community resources is essential.
