Guided by compassion, dignity, empathy, and unity, we deliver high-quality, culturally sensitive care that promotes wellness and belonging for all.The Family Practice Physician provides comprehensive, patient-centered primary care services to individuals and families across the lifespan within a Federally Qualified Health Center Look-Alike (FQHC Look-Alike) setting. This role is dedicated to serving a diverse and often underserved population, with a strong emphasis on preventive care, chronic disease management, and whole-person health. The Physician works collaboratively within an interdisciplinary care team to deliver high-quality, culturally responsive care aligned with FQHC Look-Alike mission and values.Key ResponsibilitiesProvide full-spectrum primary care services, including diagnosis, treatment, and management of acute and chronic conditions for patients of all genders and agesDeliver preventive care services, including annual exams, immunizations, screenings, and health educationManage chronic diseases such as diabetes, hypertension, and asthma using evidence-based guidelinesPerform routine in-office proceduresProvide women's health services, including contraceptive counselingCoordinate care with behavioral health, dental, and specialty providers as part of an integrated care modelUtilize electronic health records (EHR) to document patient encounters accurately and timelyParticipate in care team huddles, case conferences, and quality improvement initiativesSupport population health efforts, including outreach and care gap closureProvide culturally competent, trauma-informed care to a diverse patient populationParticipate in after-hours call rotation as neededFQHC Look-Alike-Specific ExpectationsDemonstrate commitment to serving underserved and vulnerable populationsLearn, understand, and support FQHC Look-Alike compliance requirements, including UDS reporting and HRSA guidelinesContribute to achieving quality metrics and value-based care goalsMaintain productivity expectations while prioritizing quality and patient-centered careEnsuring continuity of careMinimum QualificationsMD or DO from an accredited medical schoolCompletion of an accredited Family Medicine residency programBoard certified or board eligible in Family MedicineActive and unrestricted medical license in WIDEA certificationBasic Life Support (BLS) certificationPreferred QualificationsPrior experience working in an FQHC, community health center, or underserved settingBilingual skillsExperience with integrated care models (behavioral health, dental coordination)Familiarity with value-based care and quality metricsCore CompetenciesStrong clinical judgment and diagnostic skillsCultural humility and commitment to health equityAbility to work effectively in a team-based, fast-paced environmentExcellent communication and patient engagement skillsAdaptability and problem-solvingWork Environment & ScheduleOutpatient clinic setting within an FQHC Look-AlikeTeam-based care model with support staffFull-Time - 4 days/32 hours per weekWhy Join UsMission-driven work serving the communityCollaborative, team-based environmentOpportunity to make a meaningful impact on health equityEligibility for loan repayment programs (NHSC)Benefits – We offer professional liability insurance coverage and holiday pay to ensure you are taken care of both at work and in life.
