Speech Language Pathologist (SLP)

Birchwood Terrace Rehabilitation and Healthcare is seeking a compassionate and dedicated Speech Language Pathologist (SLP) to join our rehabilitation team in Burlington, Vermont .

As a Speech Language Pathologist at Birchwood Terrace, you will provide individualized, person-centered therapy to residents and patients while working alongside a collaborative interdisciplinary team. Speech Language Pathologist Responsibilities

• Perform comprehensive evaluations to assess patients' speech, language, communication, cognitive, and swallowing needs.

• Develop individualized treatment plans based on evaluation findings, patient needs, and functional goals.

• Provide evidence-based speech, language, cognitive, and swallowing therapy interventions.

• Establish realistic and measurable short- and long-term treatment goals.

• Monitor patient progress and modify treatment plans as clinically appropriate.

• Provide education and training to patients, families, caregivers, and other members of the care team.

• Collaborate with nursing, therapy, medical, and other interdisciplinary team members to coordinate patient care.

• Maintain accurate, complete, and timely documentation in accordance with facility policies and applicable state and federal regulations.

• Participate in care planning and interdisciplinary meetings as appropriate.

• Communicate effectively with patients, families, caregivers, and members of the healthcare team.

• Perform other duties as assigned by the Director of Rehabilitation.

Speech Language Pathologist Qualifications

• Master's or doctoral degree in Speech-Language Pathology from an accredited program.

• Current and valid Speech-Language Pathologist license in the State of Vermont .

• Must be in good standing with all applicable licensing and regulatory agencies.

• Certificate of Clinical Competence (CCC-SLP) preferred.

• Previous experience in skilled nursing, long-term care, or geriatric rehabilitation is preferred but not required .

• Strong clinical, communication, organizational, and interpersonal skills.

• Commitment to providing compassionate, patient-centered care.
