What You Will Do
* Practice independently in the community as part of an interdisciplinary care team.
* Conduct comprehensive patient assessments and develop individualized treatment plans.
* Deliver personalized home-based physical therapy services.
* Monitor patient progress and adjust plans of care as appropriate.
* Educate patients and caregivers to support recovery and independence.
* Collaborate with physicians and interdisciplinary team members.
* Manage your own flexible schedule while meeting Guaranteed Flex Pay role expectations.
* Utilize innovative technology to efficiently document patient care.
Qualifications
* Minimum of one (1) year of Physical Therapy experience preferred, or successful completion of the VNS Health Rehab Intern program.
* Current New York State Physical Therapist License and Registration required.
* Valid driver‘s license may be required based on assigned territory.
Pay Range per Visit:
* Bronx & Westchester: Weekday visits - $105 | Weekend visits - $110
