• Conduct comprehensive biopsychosocial assessments for hospice patients and their families, identifying emotional, social, and psychological needs.

• Provide individual and family counseling through evidence-based practices, fostering therapeutic relationships while understanding the unique dynamics of end-of-life scenarios.

• Develop tailored service plans aligned with patient goals, coordinating with other healthcare professionals to ensure integrated care.

• Advocate vigorously for patients’ rights and access to necessary resources, engaging with community services and ensuring all social justice considerations are addressed.

• Participate in program evaluation and development, contributing insights for grant reporting and best practices within the hospice sector.

• Engage with community organizations, government agencies, and volunteers to enhance service offerings and strengthen resource availability for patients.

• Foster a supportive environment by leading training sessions, peer discussions, and community outreach initiatives to enhance understanding of hospice services.

As a Licensed Clinical Social Worker at Mice Healthcare, you will primarily serve hospice patients, who often face complex emotional and social challenges related to terminal illness. Your support will extend to their families, helping them navigate the often-difficult conversations surrounding end-of-life care while providing emotional stability and guidance during this sensitive time.

• A Master’s degree in Social Work from an accredited institution is required.
\n
• Possession of an active state licensure as a Licensed Clinical Social Worker (LCSW).

• Demonstrated understanding of trauma-informed care principles and social justice issues that intersect with hospice care.

• Exemplary communication, empathy, and organizational skills, capable of fostering strong relationships in a remote work setting.

• At least one year of relevant experience in nonprofit healthcare settings or environments closely aligned with social work and hospice care.

The compensation for this position ranges from USD 75,000 to 100,000 annually, commensurate with experience.
