Newfane Rehab and Health Care Center on Transit Road is hiring a Registered Dietitian. If you're a Registered Dietitian (RD) or Clinical Dietitian who would rather know your residents by name than work through a stack of charts you never connect to a face, this Registered Dietitian position is worth a look. We're paying up to $80,000 for the right Registered Dietitian. We've been caring for families in Newfane and across Niagara County for close to 50 years, and "At Newfane, you are family" isn't a line we put on the website and forget about. We're a community-style building — short-term rehabilitation, long-term care (LTC), 24-hour skilled nursing, respiratory and tracheostomy care, wound care, hospice and palliative care, and respite care, all under one roof — with a multidisciplinary team that builds an individualized plan of care around each resident. Nutrition is a real part of that plan here, not an afterthought, and the Registered Dietitian we hire will have a genuine seat at the table with nursing, therapy, and food service. What You\'ll Do Complete nutritional assessments and reassessments on new admissions, short-term rehab patients, and long-term care (LTC) residents Write and update individualized nutrition care plans and document per federal and New York State requirements Complete the nutrition sections of the Minimum Data Set (MDS) and participate in care plan meetings with the interdisciplinary team (IDT) Manage clinical nutrition: enteral nutrition and tube feedings, wound healing and pressure injury nutrition, unplanned weight loss, dysphagia and altered-texture diets, renal and diabetic diets Work directly with the kitchen on therapeutic diet orders, menu accuracy, and substitutions Meet with residents and families about diets, preferences, and goals — the part of the job that makes the rest of it work Keep the nutrition program survey-ready Qualifications Registered Dietitian (RD) or Clinical Dietitian — required Clinical dietitian experience is a plus Nursing home, skilled nursing facility (SNF), or long-term care (LTC) experience is a plus — not required.
