Job Summary

We are looking for a Home Care Aide/ Caregiver/ DSP to join our team! You will be directly working with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those we serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health Care experience is a plus. 

Responsibilities 

• Support people to achieve personal outcomes that characterize a high quality of life
• Understand. Follow through, and document support plans, goals, activities and progress.
• Support clients with daily living skills and personal support needs.
• Facilitate activities that provide clients with social capital and connections to the community.
• Maintain confidentiality and support privacy at all times.
• Administer medications accurately and reliably.
• Monitor medical conditions thoroughly and consistently
• Ensure that clients are free from abuse, neglect and exploitation.
• Complete necessary reporting and documentation on time.
• Support clients with daily living skills and personal support needs
• Follow outlined care plans and financial supports 
• Report expenses on ledger and assist caregivers with client financial allowances.
• Be a safe and conscientious driver when operating company vehicles or transporting clients.
• Performs other related duties as assigned.

Qualifications

• High school or equivalent (Required) 
• Ability to pass criminal/registry background checks
• At least one year prior professional experience 
• Driver's License (Required) 
• Ability to pass and maintain required trainings such as CPR/First Aid, Medication Administration and Positive Behavioral Support Training
• Must have exceptional communication skills including written, verbal, etc.
• Must possess superior judgment, listening, and decision-making skills
• Ability to work with a variety of personalities.
