The Johns Hopkins Center for Substance Use and Infectious Disease Care Integration is seeking a full-time Sr. Clinical Nurse . The candidate we are seeking must be highly motivated, detail-oriented, and proactive to support implementation research, programmatic, and clinical objectives in a hybrid role working with The Johns Hopkins Center for Substance Use and Infectious Disease Care Integration and the Frederick County Health Department’s Street Safe Program. Integrated into the Frederick County Health Department’s Street Safe Program and its services, the Sr. Clinical Nurse will assume the role of clinician champion for an evidence-based intervention (SPRUCE) to increase uptake of HIV self-testing and prevention medication (PrEP) by PWUD through leveraging social networks and an innovative peer training program developed in collaboration with community advisory boards (CABs). The intervention is being conducted in two rural Maryland harm reduction programs, and the Sr. RN is physically based in the Frederick County Health Department’s Street Safe Program (SSP). The Sr. Clinical Nurse is an integral part of a multidisciplinary team of Johns Hopkins telemedicine-based researchers and clinicians, Peer Recovery Specialists, community partners, public health professionals, and rural Maryland Health Departments focused on reducing unintentional harms of drug use, engaging and treating persons who use drugs (PWUD) for SUD and related infectious diseases through tight care coordination, nursing assessments, and nurse case management services. The Clinical Nurse will have primary responsibility for making long-acting injectable medications (LAIs) used to treat Opioid and Alcohol Use Disorders and prevent HIV readily accessible to patients in designated sites. Maintenance of a visible and engaging on-site and community presence; coordinating health care services for PWUD including SUD treatment, screening for infectious diseases, prevention and treatment of wounds, HIV, and hepatitis C; management of laboratory supplies and testing including on-site and coordinating events; procurement and management of specialty medications; test results monitoring and timely clinical communication; nursing case management; and community outreach. The Sr. RN is responsible for supporting implementation projects centering the engagement and wellness of PWUD. They will deliver structured interventions to research and program participants and expand telemedicine services beyond the SSP to community sites where PWUD receive care. They will champion the successful integration of SPRUCE into existing services in a manner compatible with site workflows and resources; with the peer recovery specialist, conduct intervention sessions, provide direct nursing services, and facilitate uptake of HIV Pre-Exposure Prophylaxis among PWUD. Specific Duties & Responsibilities Warm and persistent engagement of PWUD, active collaboration with community stakeholders. Maintain visible on-site presence and publicize availability of services through active and ongoing relationship-building with individuals and stakeholders.’ Serve as a resource for other staff regarding clinical and research protocols and standard operating procedures. Attend CAB meetings. Actively recruit SPRUCE study participants from clients of the SSP. Deliver structured interventions to study and program participants. Establish a clear understanding of the mission, purpose, and workflow of the SSP as well as a clear understanding of the SPRUCE project deliverables, interventions, and trainings to champion successful implementation of SPRUCE. Primary responsibility for laboratory and testing programs including maintenance of unexpired supply/inventory, rapid and confirmatory testing program coordination, specimen collection and processing, and monitoring for and acting on results promptly. Proficient phlebotomy/venipuncture for PWUD in a manner that is accessible, non-stigmatizing, and acceptable to patients. Close collaboration with on-site and remote multidisciplinary teams. Nursing intake for new patients, EMR (Epic) documentation, coordinate labs, and timely scheduling of provider telemedicine appointments. Comprehensive nurse case management services in-person or via telephone. Primary responsibility for receiving specialty medications, maintaining inventory logs, ensuring proper storage, and communicating with partner pharmacies. Direct coordination between Johns Hopkins Specialty Pharmacy, insurance, and prior authorization activities needed to achieve medication approval and proper delivery. Primary responsibility for tracking prescription due date and scheduled required laboratory tests. Monitor and communicate results of lab, radiology, and diagnostic orders before, during, and after treatment to ensure that protocols are followed and continuity of care provided. Advocate for patients, coordinate care with insurance companies and outpatient pharmacies to ensure care needs are met. Coordinate ordering, proper storage, inventory, and administration of LAIs. Wound care provision. Perform regular follow-up visits with patients in-person or via telephone for medication education, adherence, radiology and lab appointments, referrals, and to address barriers to care posed by Social Determinants of Health. Counsel patients regarding harm reduction/disease prevention, health promotion, and treatment plans. Alongside peer recovery specialists and outreach workers, promote available services to prospective patients, health care providers, addiction treatment programs, and community organizations. Travel to partner sites, including where people are re-entering the community from incarceration, Opioid Treatment Programs, residential recovery programs, encampments, and other sites where PWUD are. Maintain regular communication with referring providers and agencies to ensure tight care coordination to optimize patient and referring provider/agency experience. Document clinical and research activities accurately and in a timely manner. Attend meetings in-person at Johns Hopkins in Baltimore as required for educational and programmatic purposes. Provide clinical care up to the scope of a registered nurse in the state of Maryland. Must have reliable transportation. Mileage reimbursement will be provided at the Federal mileage reimbursement rates. Other duties as assigned. Minimum Qualifications Individual must be a registered nurse, licensed in the State of Maryland or state where practicing. Bachelor's degree in nursing or related discipline required. Current CPR certification required. Must maintain current licensure and certification during duration of employment. Require minimum of two years of related nursing experience, with additional experience preferred. Additional education may substitute for required experience and additional related experience may substitute for required education beyond a high school diploma/graduation equivalent, to the extent permitted by the JHU equivalency formula. Preferred Qualifications Proficiency in the Epic electronic medical record system and MS Office Basic data management skills. Demonstrated high level of motivation, strong organizational skills, and attention to detail. Flexible stance toward service provision that centers PWUD and working as part of a multidisciplinary team. Capacity to model and maintain an intentionally de-stigmatizing and welcoming environment for PWUD. Knowledge and interest in HIV, HCV, substance use disorder management and treatment, and public health aspects of infectious disease control. Self-starter with the ability to work with minimal supervision. Technical Qualifications & Specialized Certifications Must attend trainings in and qualify for the use of electronic medical records, scheduling, and other Hopkins and FCHD employer-required trainings. Certification as a phlebotomist per Maryland State requirements within 6 months of employment, with ongoing proficiency training/maintenance of skills expected. Employer to cover cost of training.
