Job Profile Summary The RN, Nights and Weekends provides after‑hours field visits to patients, families, and community partners. The role establishes and maintains positive relationships with team members, patients/families, and referral resources. Qualifications Current RN license in the state where the employee will be working Minimum one year of nursing experience; hospice or hospital experience preferred Previous experience working with an EMR/EHR system Effective communication in English (verbal and written); bilingual in Spanish preferred Valid driver’s license and automobile insurance per Company policy; reliable transportation to meet patient needs Excellent critical decision‑making and problem‑solving skills Excellent clinical and assessment skills Ability to use equipment with visual and auditory mechanisms Excellent time‑management skills with the ability to prioritize demands to meet patient service standards and deadlines Can handle stressful situations professionally and calmly while providing exceptional customer service Strong patient service skills and information‑seeking / probing skills Accurate knowledge transfer, listening, understanding, and responding Navigation of a fast‑paced environment using interpersonal skills during patient visits Cooperative team‑work skills and a sense of urgency in daily work activities Computer operations skills; proficiency in MS Office, online research, and data entry Availability to work shifts to accommodate extended hours of operation Ability to perform sedentary work with frequent interruptions Capacity to manage emotional stress while caring for terminally ill patients and families Physical ability to lift or move patients and equipment, bend, push/pull, and kneel, with or without reasonable accommodations Active BLS certification for healthcare professionals (American Heart Association or Red Cross) Responsibilities Makes after‑hours visits to patients in their home, assisted living facility, nursing home, or hospital within the assigned territory Provides compassionate customer service to patients and families Assesses patient/family, identifies immediate needs and appropriate nursing interventions by phone and/or during visit Provides and manages direct care as part of an Interdisciplinary Group (IDG), incorporating psychosocial, spiritual, cultural, physical, and biological components Demonstrates clinical skills regarding case management, application of company policy and procedure, physical assessment, intravenous therapies, pain medication titration, symptom management, and patient/family relationships Prioritizes, consults, refers, and collaborates with other professionals to promote quality and continuity of patient/family care Educates patients, families, caregivers, and other health professionals about disease process and decline, palliative interventions, caregiving, dying process, and safety practices Reports changes in the patient’s condition to appropriate members of the IDG or other health professionals Attends patient deaths; prepares body for transport, oversees disposal of medications, and provides bereavement support Completes documentation per policy and procedure During emergencies (e.g., hurricanes) may be required to report to a designated location to ensure continuity of services, possibly staying overnight based on duration of emergency Performs other duties as assigned Compensation Pay Range: $67,976.53 – $106,213.33 Compliance This position requires consent to drug and/or alcohol testing after a conditional offer of employment is made, and ongoing compliance with the Drug‑Free Workplace Policy.
