Social Worker LCSWThe Social Worker LCSW is fully engaged in providing quality/no harm, customer service and stewardship by utilizing advanced psychosocial skills to facilitate the coordination of patient care through assessment, planning, intervention and evaluation. The Social Worker LCSW uses current knowledge to effectively evaluate, promote and utilize hospital and community resources and services to achieve optimal outcomes, serving as a role model for coordination of psychosocial services for assigned patients in collaboration with the patient, family, physician, patient care team, and payers.Primary responsibilities include:Collaboratively determining desired discharge goals and communicating all discharge plans and options to patient/family, physicians and staff members.Providing counseling and researching placement alternatives for substance abuse, mental health, homeless and indigent patients.Facilitating and participating in transfer of Baker Act and Marchman placements to mental health facilities. Providing therapeutic interventions with patients and families as needed.Completing mandatory reporting processes.Facilitating referrals for post-discharge psychosocial and therapeutic needs.In a crisis situation where safety of the patient or others is in immediate jeopardy, the LCSW can Baker Act a patient.Mentoring social work students from local universities during their internship in the department.Providing clinical supervision for social workers in the department and those pursuing their LCSW.Identifying system/process breakdowns and improvement opportunities, documenting for the Performance Improvement Plan.Intervening and recommending appropriate referrals to resolve system barriers to patient progression, collaborating with other disciplines as directed.Communicating avoidable delays to the RN Case Manager for collaboration and documentation in MIDAS.Ensuring compliance with Federal and state regulatory standards of care, identifying risk management and quality issues, and intervening or referring as indicated.Minimum qualifications include:Education: Masters of Social WorkLicensure: Current Florida LCSW licenseCertification: Current American Heart Association Basic Life Support Healthcare Provider Completion Card upon hire and maintainedWork Experience: One (1) year of previous social work experienceKnowledge/Skills/Abilities:Basic computer skills including knowledge of MS Office Outlook and WordCurrent knowledge of healthcare economic issues impacting acute careWorking knowledge of social work scope of practice and current issues impacting healthcareUnderstanding of payer/reimbursement practices and regulationsAbility to understand and comply with Federal and state regulatory standards of careAbility to effectively handle multiple demands in a short timeframePreferred qualifications include:Certification: CCM or ACM Case Management CertificationWork Experience: Case Management experience in Hospital settingPhysical requirements (Sedentary):Majority of time involves sitting or standing; occasional walking, bending, stoopingLong periods of computer time or at workstationLight work that may include lifting or moving objects up to 20 pounds with or without assistanceMay be exposed to inside environments with varied temperatures, air quality, lighting and/or low to moderate noiseCommunicating with others to exchange informationVisual acuity and hand-eye coordination to perform tasksWorkspace may vary from open to confinedMay require travel to various facilities within and beyond county perimeter; may require use of personal vehicleAbout Health First:At Health First, diversity and inclusion are essential for our continued growth and evolution.
