Overview:

1. Coordinate and manage care plans for long-term care residents
2. Serve as a liaison between residents, families, and healthcare providers
3. Ensure compliance with all regulatory and company standards
4. Monitor resident outcomes and support care teams in achieving goals
5. Provide guidance, education, and advocacy for residents and families

Requirements:

• Active RN or LPN license in the state of Ohio in good standing
• Minimum 3 years experience in long-term care, skilled nursing, or case management required
• Strong communication, organizational, and problem-solving skills
• Comfort working independently in a remote environment
• Compassionate, resident-centered mindset
