Our platform streamlines documentation and compliance so providers can focus on delivering high-quality care to seniors.

We are seeking a Primary Care Physician (MD/DO) who owns an existing clinic to provide in-facility care to residents in assisted living communities across the Inland Empire, CA. This role involves mobile in-facility visits with clustered routes to minimize travel time. This is a part-time position starting at a minimum of 1 day per week (Monday through Thursday), with potential to expand based on patient panel growth. This position is available as W-2 or 1099 we are happy to accommodate what works best for you.

Responsibilities include:

• Providing comprehensive primary care to seniors in assisted living and RCFE communities in the Inland Empire area
• Conducting in-facility patient visits and telehealth consultations as needed
• Managing chronic conditions, preventive care, and acute concerns
• Collaborating with our care team including NPs, specialists, and facility staff
• Maintaining accurate and timely documentation in compliance with regulatory requirements
