Your Role: * * Deliver personalized physical therapy services to home-based clients. * Conduct initial and ongoing assessments to track progress and adjust treatment plans. * Perform fall risk assessments at Assisted Living Facilities (ALFs) and community expos. * Supervise client care to ensure best practices and quality outcomes. * Represent our organization at ALF events and expos, providing educational presentations. * Travel to client locations and events as needed. * Supervise Home Care clients and caregivers * Create and update care plans for non-medical clients * Administrative duties including calling PCP offices for orders What You Need: * Licensed Physical Therapist (PT) in the state of Hawaii. * 1+ years of experience in a clinical or home-based care setting. * Experience with fall risk assessments (preferred). * Excellent communication and interpersonal skills to engage clients and the community.
