Key Responsibilities\n

  • Conducting thorough assessments and evaluations to understand the psychological, emotional, and social dynamics affecting clients and their families.

  • Facilitating the development and implementation of Individualized Family Service Plans (IFSP), ensuring services are tailored to meet the unique needs of each family.
  • \n
  • Engaging with family members to offer emotional support, coping strategies, and practical resources to navigate the complexities of hospice care.
  • \n
  • Monitoring client progress and adjusting care strategies in collaboration with the healthcare team to achieve optimal health outcomes.
  • \n
  • Adhering to all federal and state regulations, safeguarding the interests and well-being of our clients at all times.
  • \n
  • Creating a nurturing rapport with clients and their families to foster a trusting environment conducive to healing and acceptance.
  • \n
  • Implementing evidence-based practices and natural environment teaching methods to enhance family involvement and child development where applicable.

  • Encouraging families with skill-building activities to promote their child’s emotional and developmental growth, utilizing the coaching model in your practice.

Patient Population / Client Focus\n

In this role, you will primarily serve families navigating the complexities of end-of-life care, focusing on children and their parents during what can be an emotionally challenging time. You will work closely with families to provide psychological support, guidance, and community resources to ease their transition and enhance their overall well-being. Your expertise will play a crucial role in helping families maintain quality of life amidst difficult circumstances.

Qualifications\n

  • A Master’s degree or Doctorate in Social Work is essential.

  • Current licensure as a Licensed Clinical Social Worker (LCSW) by the Department of Public Health.

  • Possession of a valid driver’s license for potential in-person consultations.

  • Demonstrated experience in social work, particularly in hospice or related environments.

Compensation and Benefits\n
