Crowne Health Care is currently accepting applications for a Restorative Nurse (RN or LPN) within our long-term care and rehabilitation facility specializing in geriatric care.

We are seeking a dependable, organized, and compassionate nurse who is committed to helping residents maintain or improve their highest possible level of function and independence. This position works closely with nursing, therapy, MDS, and administration to coordinate restorative nursing services and ensure accurate documentation.

Qualifications

• Current, active RN or LPN license
• Long-term care or skilled nursing experience preferred
• Restorative nursing experience preferred
• Strong communication, organization, and documentation skills
• Ability to work collaboratively with nursing, therapy, MDS, and facility leadership

Essential Job Functions

• Coordinate and supervise restorative nursing programs and services for residents
• Monitor residents’ progress and ensure restorative programs are being carried out according to the plan of care
• Work closely with therapy staff to help residents achieve and maintain their highest practicable level of function
• Collaborate with MDS, nursing, therapy, and administration regarding resident care and restorative needs
• Ensure restorative nursing documentation is complete, accurate, and timely
• Assist with maintaining appropriate documentation to support care planning, reimbursement, and regulatory requirements
• Educate and support nursing staff and CNAs regarding restorative programs and techniques
• Monitor restorative programs and make recommendations for changes based on resident progress and needs
• Maintain compliance with applicable federal, state, and facility policies and procedures
• Promote resident dignity, independence, mobility, and quality of life
