Nurse Practitioner OpportunityPine Park Health is looking for a Full-Time Nurse Practitioner responsible for delivering care in our San Francisco Peninsula market (Palo Alto, Redwood City, San Mateo). This person-centered care model works to leverage the power of our care navigators to maximize the time and energy of our providers.What You'll Do:Provide onsite primary care services to older adults living in Senior Living CommunitiesDevelop and execute detailed person-centered care plans on all primary care patientsWork with Medical Director to identify and develop policies to address issues that arise which affect day to day practice, culture or clinical guidelines, including for emerging disease(s)Provide monthly Chronic Care Management (CCM) visits and care; review and sign off on CCM documentationRun regular huddles and provide leadership to the clinical teamIdentify ways of working with senior living management and care teams to better carry out Pine Park care plans and improve patient outcomesQualifications:Nurse Practitioner - preferably with geriatrics experienceComfortable with a fast-paced and changing environmentWillingness to provide onsite clinical across the San Francisco Peninsula regionDesire to work with complex care patients and support the building of a holistic care modelStrong orientation towards collaboration and working closely with a multidisciplinary team; values input of all members of the clinical teamStrong written and verbal communicatorAbility to work with tech-enabled care delivery tools (e.g., project management, 'checklist' management, remote monitoring, etc.) used together to improve patient care and team functionFlexible work schedule with travel within the San Francisco Peninsula area. The final compensation package for each successful candidate will depend on several job-related factors unique to each candidate. These factors may include but are not limited to education, training, skill set, years and depth of experience, certifications and licensure, business needs, internal peer equity, and alignment with geographic and market data.
