Job Description:
Aventura at West Park is seeking a dedicated and compassionate Full-Time Physical Therapist Assistant (PTA) to join our rehabilitation team. The PTA will work under the supervision of a licensed Physical Therapist to help residents regain mobility, improve functional independence, and enhance overall quality of life.

Responsibilities:

• Provide physical therapy treatments in accordance with established care plans

• Assist the Physical Therapist in implementing individualized treatment programs

• Monitor and document patient progress and response to treatment

• Communicate effectively with interdisciplinary team members

• Maintain accurate and timely documentation in compliance with facility and regulatory guidelines

• Ensure a safe and supportive environment for all residents

Qualifications:

• Associate degree from an accredited Physical Therapist Assistant program

• Current Ohio PTA license in good standing

• Previous experience in a SNF or LTC setting preferred

• Strong communication and interpersonal skills

• Ability to work effectively as part of a team
