Home Health Physical Therapist

This is a 13-week contract position.

Day shift, 5 days a week, with a typical schedule of 08:00 - 17:00.

We are seeking an experienced Home Health Physical Therapist to provide skilled care to patients in their homes. The ideal candidate will demonstrate expertise in various areas including assessment, documentation, and the use of specific equipment. The Physical Therapist will be responsible for developing and implementing comprehensive treatment plans to improve patients' mobility and overall functionality.

Responsibilities

• Conduct thorough patient assessments to determine needs and create appropriate treatment plans.
• Implement evidence-based therapeutic interventions and progress tracking.
• Educate patients and their families on exercises and strategies to enhance recovery.
• Maintain accurate documentation and compliance with regulatory standards.
• Utilize adaptive devices and equipment effectively in patient care.
• Coordinate care with interdisciplinary team members and facilitate communication.

Required Experience / Certifications / Licensure

• 1 year of experience in home health or similar settings.
• Must hold a valid state license to practice as a Physical Therapist.
• Current BLS certification is required.
• OASIS training is preferred.
• Must have a valid driver's license and reliable transportation for home visits.
