Weekend Warrior | Licensed Practical Nurse (LPN)

Countryside Manor is hiring Licensed Practical Nurses (LPNs) for our Weekend Warrior program in Fremont, Ohio!

Are you an LPN looking for a weekend nursing opportunity where you can feel supported, appreciated, and make a difference? Join our nursing team at Countryside Manor and provide compassionate, quality care to our residents.

Experienced LPNs and newly licensed nurses are encouraged to apply!

LPN Responsibilities

Provide direct nursing care to residents in a skilled nursing and long-term care setting
Administer medications and treatments as ordered
Monitor residents and assess changes in condition
Document resident care accurately and timely
Communicate changes in resident condition to physicians and appropriate members of the care team
Follow individualized resident care plans
Provide guidance and support to CNAs and other members of the nursing team
Respond promptly to resident needs and concerns
Maintain resident dignity, privacy, comfort, and safety
Follow infection control, safety, and facility procedures
Communicate effectively with residents, families, physicians, and healthcare team members
Work collaboratively with the nursing team to provide excellent resident care

LPN Qualifications

Current Licensed Practical Nurse (LPN) license
Compassionate, dependable, and resident-focused
Strong clinical, communication, and teamwork skills
Ability to meet Weekend Warrior scheduling requirements
Long-term care, nursing home, or skilled nursing experience is a plus, but not required
