Quality - Physician Assistant - Virtual Care for Chronic Conditions (VC3)

Job Summary Kaiser Permanente-Washington Permanente Medical Group is seeking a full‑time 1.0 FTE Primary Care Physician Assistant to join our Quality, Documentation & Reporting Team. The VC3 (Virtual Care for Chronic Conditions) Clinician will ensure quality care for identified WPMG patients using virtual encounters to address conditions and close care gaps in a patient‑centered manner. They will preview, update, and modify the health record in HealthConnect to optimize the chart for virtual encounters and perform PC3 (Patient Centered Charting and Communication) chart reviews between visits. Work will be monitored through proficiency metrics and chart audits to ensure documentation quality, and VC3 Clinicians will report to the Assistant Medical Director of Documentation and Reporting. Principal Duties & Responsibilities Conduct on‑demand virtual visits for adult members during normal clinic hours Monday through Friday and perform chart reviews between visits to ensure completeness and accuracy of the medical record according to PC3 Clinician Reviewer standards. Maintain an FTE of 1.0; any reduction in clinical FTE requires approval by Primary Care leadership (AMD & DMD). Demonstrate proficiency with care gap closure and provide accurate, complete documentation of disease‑state management. Provide expert integrated care and diagnostic services in the virtual setting; coordinate care remotely across KPWA and uphold remote standards of care. Work remotely in compliance with all WPMG & KPWA remote work policies; clinicians may work in a KP facility if a remote connection is not available, provided this does not impact in‑person operations. Collaborate with Primary Care, Specialty Care, and THMC to improve care coordination for members. Perform PC3 chart reviews between virtual visits by reviewing multiple sections of the medical record, updating the Problem List and MCR Best Practice Advisory, consolidating diagnoses, and prioritizing the Problem List per PC3 guidelines. Maintain flexibility in staffing, including arranging vacation/PTO in coordination with other VC3 providers to ensure adequate coverage. Ensure an appropriate work‑from‑home environment with a professional background, quiet space, and privacy to safeguard patient confidentiality. Work effectively with an inter‑disciplinary team to explore patient health conditions, diagnose issues, and provide positive patient experiences; maintain cultural competency awareness. Analyze reports, test results, medical records and examinations to diagnose conditions, propose treatment options, and order appropriate laboratory, radiology, medication or referral services. Review and update medical records, including current medications, allergies, medical/surgery history, to ensure accurate information and quality care. Advise patients on lifestyle and diet changes that may improve health or treat specific conditions. Adhere to departmental policies, procedures, safety, environmental and infection control standards. Maintain patient confidentiality and comply with all federal and state health information privacy laws. Record complete, timely electronic medical records that meet documentation and coding standards. Be available to work in at least half‑day increments (0.1 FTE). Qualifications Education : Physician Assistant or equivalent. Licenses & Certifications : PA‑C (NCCPA), DEA License, WA Medical License; unrestricted Washington State Medical License and federally issued DEA license are required. Experience : Must possess a full range of clinical skills and interest in working with an innovative group practice. Preferred : Proficiency in using HealthConnect, routine use of the ambulatory instance of HealthConnect, and membership in Family Medicine, Internal Medicine, Pediatrics or a similar broad‑scope specialty; good clinical standing with active licensure and board certification in the specialty.
