Job Details

Key Responsibilities:
Provide comprehensive, patient-centered preventive care, including routine physicals, health-risk assessments, immunizations, and screenings.
Coordinate patient care across multiple healthcare settings and help ensure smooth transitions of care.
Prescribe and manage medications, monitor effectiveness, and make adjustments based on patient needs.
Develop and implement strategies focused on population health, disease prevention, health promotion, and chronic disease management.
Collaborate with physicians and other healthcare professionals to develop, implement, and evaluate patient care plans.
Educate patients and families on disease prevention, healthy lifestyle choices, and ongoing health maintenance.
Utilize clinical data and population health strategies to improve patient outcomes.
Participate in quality improvement and research initiatives designed to enhance patient care.
Maintain accurate documentation within the Electronic Health Record (EHR) system.
Effectively manage multiple patients, priorities, and responsibilities in a fast-paced clinical environment.

Experience & Qualifications:
Current, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license.
1+ years of experience as a Nurse Practitioner or Physician Assistant.
Demonstrated experience with:
Preventive care
Care coordination
Medication management and prescribing
Patient education
Strong clinical decision-making, analytical, and problem-solving skills.
Strong commitment to patient-centered care and improving community health.
