About the Role

HouseCalls brings preventive care directly to health plan members through annual in-home visits. As an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) , you'll perform comprehensive health assessments that help identify unmet needs, support ongoing care management, and improve overall health outcomes.

This is a non-prescribing, field-based position focused on building meaningful patient connections, providing health education, and partnering with primary care providers to ensure members receive the care and support they need.

Primary Responsibilities

• Complete comprehensive in-home health assessments, including:
  • Medical history review
  • Medication reconciliation
  • Vital signs and physical examinations
  • Evidence-based screenings and point-of-care testing, when appropriate
• Identify and document clinical conditions to support care planning
• Share assessment findings with primary care providers to help close gaps in care
• Recognize urgent or emergent situations and take appropriate action
• Educate patients on disease management, medications, and treatment adherence
• Assess social determinants of health and connect members with community resources as needed
