The Home Health Aide (HHA) provides hands-on personal care and emotional support to clients in their homes under the supervision of a Registered Nurse. This role supports independence, dignity and safety while delivering compassionate, high-quality care aligned with Dignity At Home’s core values. What You’ll Do: • Assist with activities of daily living including bathing, grooming, dressing and toileting • Support medication self-administration and proper storage • Monitor vital signs, weight and report changes in condition • Provide catheter care, ostomy care and assist with prescribed medical equipment • Perform light housekeeping, laundry and meal preparation • Accompany clients to appointments and provide companionship • Follow individualized care plans and document care accurately each shift • Communicate changes in client condition to the Clinical Manager promptly What We’re Looking For: • Active New York State Home Health Aide certification • Completion of an approved HHA Training Program • At least 18 years of age (if required by state regulations) • Strong communication skills and ability to follow care plans • Reliable transportation and flexibility for coverage as needed • Compassionate, dependable and committed to client well-being Home Health Aide – Provide Compassionate Care and Daily Support Home Health Aide | Personal Care Assistant | Senior Care Pay Range: 19.75 - 24.75 Ready to take the next step and make a bigger impact than you ever imagined?
