Key Responsibilities

• Conduct comprehensive examinations and evaluations of patients presenting with functional limitations and disabilities. Develop and implement individualized treatment plans based on assessment findings.
• Adhere to all relevant state practice acts and regulations governing the professional practice of physical therapy.
• Perform evaluation services that encompass reviewing patients' medical histories and conducting specialized assessments to inform treatment decisions.
• Implement treatment protocols derived from evaluation data, making modifications as needed based on the patient's progression and feedback.
• Collaborate with other healthcare professionals, recognizing the importance of a multidisciplinary approach to patient care.
• Monitor patient progress and document changes in condition, ensuring that treatment plans are adjusted as necessary to optimize rehabilitation outcomes.
• Complete all necessary discharge planning and follow-up care to support patients in maintaining their progress after leaving our facility.
• Maintain accurate and timely documentation of patient interactions, including evaluations, treatment plans, encounter notes, and discharge summaries.
• Engage in ongoing education and training to comply with corporate and clinical service standards as well as state and federal regulations.
• Perform assigned treatments daily while maximizing the quality of care and departmental productivity, ensuring positive patient experiences.

Patient Population / Client Focus

You will primarily work with a diverse patient population, including individuals recovering from surgical procedures, sports injuries, neurological conditions, and age-related mobility issues. Your expertise will be pivotal in designing customized rehabilitation programs that address each patient's unique needs and facilitate their return to daily activities and sports.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited institution.
• A valid state license to practice physical therapy in New Jersey.
• A commitment to continuous professional development and maintaining current CPR certification.
• Strong interpersonal skills with the ability to build rapport quickly with patients, fostering trust and encouraging open communication.

Preferred Experience

• At least two years of experience in a clinical setting, preferably in a rehabilitation facility or outpatient clinic.
• Experience in utilizing digital applications and solutions in patient treatment and monitoring.
